Travel Vaccinations

You do not need to be registered with us to attend our travel immunisation clinic.

Mendip Country Practice is a registered Yellow Fever Vaccination Centre. We are easy to find and have car parking facilities.

As travel advice is now more complex we do ask that you complete a travel form in advance of consulting with our Practice Nurse.

Registered patients can download forms from below.  Private or unregistered patients need to collect forms from reception – return your form by handing in at reception along with a £10.00p deposit and collect 7 days later (this gives the nurse time to research vaccinations required dependent upon your destination etc). 

It is then the responsibility of the patient to make an appointment to see the Practice Nurse for any vaccinations needed. Our nurses will not be able to vaccinate you without a form being completed in advance. 

If you are not already a registered patient at this surgery, please also complete a Private Patient Registration Form.

When handing in your forms at Reception, we ask for a deposit of £10.00p to be paid for travel forms to be completed by the nurse, as this service is not an NHS service and some vaccinations do incur a fee – current costs are listed at reception.   The deposit will be refunded against the cost of the vaccination.  Please ensure you have means of payment available at the time of your vaccination, or alternatively payment made in advance of your appointment.
